For the Bridge Street Family Practice in Londonderry, a new contractor has been selected to deliver general practitioner services.
The Foyleside Family Practice will assume control, according to the Department of Health.
When Foyleside takes over services on August 1st, the Department of Health stated that nearly 5,000 Bridge Street patients will automatically register with Foyleside.
The Bridge Street Medical Centre's two offices are housed in the same structure.
Bridge Street's future had been in question ever since it informed the Department of Health's strategic planning and performance group (SPPG) in January that it intended to terminate its contract on July 31.
The Department of Health has six months from the time practices terminate their contracts to find new leadership to take over their operations.
Following the termination of its contract in December of last year, Racecourse Medical Centre in the Shantallow neighborhood of Derry is now operated by new management.
After potential new management fell through, the Western Health and Social Care Trust has now officially been named the center's new temporary contractor.
In response to the unexpected withdrawal of the GP contractor who was given the contract earlier this month, the trust is stepping in to offer an interim solution, a Department of Health spokesperson told BBC News NI. ".
Beginning in June, the new contract came into force.
Patients at the practice don't need to sign up with another one. They should keep in touch with the office as usual, and they will get a letter in the next few days informing them of their new GP arrangements, the spokesperson added.
