A plastic surgery hospital was given an inadequate rating after inspectors discovered it was using outdated tools and medications.
The Alexander Private Hospital in Chesterfield received criticism for its cleanliness from the Care Quality Commission (CQC).
The 21-bed hospital performs liposuction and breast augmentations among other procedures.

The report, which was based on an inspection that took place in February and March, also stated that staff failed to recognize potential mental health conditions that could have prevented some patients from having surgery.
After inspections in December and October 2022, it is the third time the independent hospital has received a rating of inadequate in less than a year.
The inspectors noted: "Furniture was not always clean and maintained, and not all theater areas were clean.
Equipment and items in storage were in some places dusty. In an operating room, we discovered ripped areas of a patient couch mattress that had been taped over.
"This was brought up during our prior inspection in October 2022 as a risk to efficient infection prevention and control.
"A blood pressure cuff that was visibly filthy and torn was also discovered. Several sinks had noticeable stains. ".
Additionally, the CQC inspectors discovered that some medications were out-of-date and others were not being stored properly.
Additionally, they expressed worries about poor record keeping, a lack of staff training, particularly in the area of patient safety, and a lack of documentation regarding any discharge instructions given to patients.
