A Bradford care facility has been placed under special measures after inspectors discovered that residents were frequently "not treated with dignity and respect.".
The Care Quality Commission (CQC) gave The Beeches Care Home, located on Beacon Road in Wibsey, an "inadequate" rating.
"Serious shortfalls" affecting safety and the standard of care were highlighted in a CQC report following an inspection in April.
It claimed that residents had an "overwhelming lack of choice" at the facility for elderly and dementia patients.
According to the CQC, it had explained to the residence where regulations weren't being followed and what steps needed to be taken.
The home's operator, Victorguard Care Limited, is required to explain to the CQC what steps will be taken to raise care standards at The Beeches in response to the report's publication.
Inspectors found that staff at the home did not manage medications safely, putting residents at risk of harm.
It also noted the lack of systems in place for efficiently assessing, monitoring, and reviewing risks pertaining to people's health, safety, and welfare.
According to inspectors, the hiring procedure for staff "was ineffective, and as a result, there were gaps in the recruitment forms and incomplete processes.".
According to the report, when inspectors spoke with some of the relatives, their responses were "generally positive," but those who were actually residing in the home provided "mixed" responses.
